It is best not to visit the grave until after the end of the twelve months. The Rabbis teach that the first year after the passing, the soul of the deceased is being judged, and it is a spiritually difficult time. If you feel that you have an overwhelming need to visit the grave, you may do so, but not on Yom Kippur itself.
