There is a disagreement among the Poskim. The Chazon Ish rules that there is an obligation if the balcony has the size of 4 amos by 4 amos in area, and the Mezuzah should be on the outside of the door. It is placed on the righthand side of the doorway coming back into the house. The reason is because coming back into the house is considered to be more significant than going out onto the balcony. Rav Ovadiah Yosef rules that there is no obligation to put up a Mezuzah on a balcony door, but that someone who does so will be blessed.
