Baruch Hu u’Varuch Shmo

Shalom aleichem Rabbi Lauffer. Thank you for answering my last question. It says in seforim that one says “Baruch hu u’varuch shemo” at the end of each bracha in Shemoneh Esrei. But I have heard some people say “Baruch Shemo” instead. Is either one correct halachically, or only the former? Thanks a lot.

The Shulchan Aruch writes that one should use the formula “Baruch Hu u’Varuch Shemo”. Where does “Baruch Shemo” come from? I fear that its only source is people not being careful enough when they say Baruch Hu u’Varuch Shemo and it comes out as a truncated Baruch Shemo instead.
