The mitzvah to eat matzah is on the first night(s) of Passover. During the rest of Passover instead of eating leaven bread we are to refrain from leaven. Does this mean we need to eat matzah all the time? Not really. Rather, the “bread” of Passover is matzah and not regular bread.
However, since the first and last day(s) of Passover are Yom Tov festival days, there is a special mitzvah to have two meals on every Yom Tov, night and day – and three meals on Shabbat during Passover. Since we cannot eat bread to begin the meal, we must eat matzah in place of bread for this meals. Therefore there is a mitzvah to eat matzah during Passover day when it is Yom Tov.

Yes. There are three meals on the Sabbath day: One at night, one in the morning, and one in the afternoon before sunset.
On Passover matzah is eaten instead of bread for during each of these three meals, including breakfast.

No, steamed rice is not matzah. Even baked rice cakes are not. Matzah must be made from specific grains, such as wheat, barley, rye and spelt – but not rice.
