Firstly, please accept my personal Bracha that your father be blessed with many more years in good health and nachat from his entire family!
Due to his age and his frailty, if your father is somehow able to get to Shul, he does not need to leave the Shul when the Kohanim go to bless the congregation. That means that he can stay in his place and become the recipient of the Kohanims’ blessing.
(However, you, of course, should go up to the front to bless if possible.)
